The following were reviewed as part of this investigation: patient risk, frequency analysis, labelling, and applicable manufacturing records and the lots in question were manufactured per specification without deviation. There was no patient injury reported with this event. Pursuit vascular submitted a hard copy to medwatch on 25 january 2019. The emdr team was furloughed during the government shutdown. Reference esg/cdrh help desk ticket (b)(4). Pursuit vascular was asked to make a note that repot was delayed for this reason to avoid being considered late. The report is now being resubmitted via emdr production system.
