According to the reporter, during a laparoscopic cholecystectomy procedure, the clips did not load properly and jumped off the jaw. The clips that fell into the cavity were retrieved using a grasper. A second clip applier from the same lot# was opened but the same issue occurred. A third clip applier with a different lot# was opened and used. There was no patient injury.
