It was reported that there was an issue with monopolar handle. The device malfunction was noted in sterile processing and not in a procedure. There were debris coming from the device so the staff dismantled the handle from the shaft, as per instructions for use (ifu). The knob was still loosely attached but open on the end. Then, staff tapped the device on the table a couple of times very lightly and debris fell out. This incident did not occur during surgery. There was no patient harm. Additional "informtaion" has been requested but not yet received as of this report. The malfunction is filed under aag reference (b)(4).
