According to the reporter, at the second stitch of a gap closure during a laparoscopic redo gastric bypass, it was not possible to move the suture gently through the tissue. It seems, that the barbs were upside down, so the surgeon could not pull the suture easy through the tissue and the thread broke. Another device with the same lot number was used to complete the case. There was no patient injury.
